The data presented demonstrate the feasibility of conducting a randomized controlled trial on EVT.
PURPOSE Patients with bilateral central scotomas adopt other retinal area(s) called preferred retinal locus to substitute the blind fovea. EVT is offered with the goal to improve functional vision by learning how to identify and use a more useful retinal area for fixation—a trained retinal locus—and/or to improve fixation stability with their existing preferred retinal locus. An observational study was conducted to determine whether patients change and adopt a new trained retinal locus location and/or if fixation stability improves after EVT.
METHODS Seventy-six patients with bilateral central scotomas received EVT from an experienced rehabilitation therapist. Retinal locations for repeated fixations and sizes of central scotomas of the better eye were recorded before and after EVT with a scanning laser ophthalmoscope. The position and area of the 95% confidence bivariate ellipse that defines the retinal area in which 95% of fixations occur were analyzed.
RESULTS We observed no significant difference between the size of the area of the fitted ellipses before (mean [SD], 26.4° [19.7°]) and after (mean [SD], 24.8° [20.9°]) EVT (P = .54). However, we observed a shift in the preferred retinal locus location after EVT (P < .0001). The mean (SD) shift in the position of the center of fixation after EVT was 7.5° (5.2°; range, 0.45 to 23°). This mean difference was larger than would be expected from average within-subject fixation stability.
CONCLUSIONS The location of the retinal area used for fixation changes after EVT. Fixation stability does not change after EVT. These preliminary data are yet to be confirmed with the use of a control group.

[Thésaurus Mesh]Vision oculaireRésumé : SIGNIFICANCE This research is significant because, although vision-related quality of life (VRQoL) is improved after vision rehabilitation (VR), patients with certain characteristics respond less positively on VRQoL measures, and this should inform future care.
PURPOSE The purposes of this study were to evaluate how two VRQoL questionnaires compare in measuring change in patient-reported outcomes after VR and to determine if patient characteristics or occupational therapy (OT) predict higher scores after rehabilitation.
METHODS In a prospective clinical cohort study, 109 patients with low vision completed the Impact of Vision Impairment (IVI) and the National Eye Institute Visual Functioning Questionnaire (NEI VFQ-25) before and after VR. Comprehensive VR included consultation with an ophthalmologist and OT if required. The relationships of six baseline characteristics (age, sex, visual acuity, contrast sensitivity, field loss, diagnosis) and OT were assessed with VRQoL scores using multivariable logistic regression.
RESULTS The mean (SD) age was 68.5 (19.2) years, and 61 (56%) were female. After rehabilitation, increases in scores were observed in all IVI subscales (reading [P < .001], mobility [P = .002], well-being [P = .0003]) and all NEI VFQ-25 subscales (functional [P = .01], socioemotional [P = .003]). Those who were referred to OT but did not attend and those who had hemianopia/field loss were less likely to have higher VRQoL in IVI mobility and well-being. Those attending OT for more than 3 hours were less likely to have better scores in emotional NEI VFQ. Men were less likely to have increased scores in functional and emotional NEI VFQ, whereas those with diagnoses of nonmacular diseases had higher odds of having increased scores on the emotional NEI VFQ (all, P < .05).
CONCLUSION Both the IVI and the NEI VFQ-25 detected change in patients' VRQoL after rehabilitation. Most of the patient characteristics we considered predicted a lower likelihood of increased scores in VRQoL.

PURPOSE The purpose of this study was to investigate fixation stability using laser Bessel and Gaussian beams and monitor-based images of these targets.
METHODS The right eyes of 16 participants were presented with seven fixation targets: monitor-based images of a bull's eye/cross hair, a Gaussian beam, a Bessel beam with four rings and a Bessel beam with three rings; laser Gaussian beam, laser Bessel beam with four rings, and laser Bessel beam with three rings. Participants fixated target centers for five runs, in which each run presented the seven targets for 20 seconds each. An Eye Tribe tracker sampled eye positions at 30 Hz. Standard deviations along horizontal (σx) and vertical meridians (σy) and areas of bivariate contour ellipses (BCEAs) of fixation positions were calculated, and statistical significances of target differences for these parameters were determined.
RESULTS Average σx, σy, and BCEAs ranged from 0.26 to 0.35°, 0.38 to 0.55°, and 0.78 to 1.31 degrees2, respectively. Target differences in σx (χ26 = 13.0, P = .04), (σy) (χ26 = 36.819, P < .001), and BCEA (χ26 = 34.406, P < .001) were statistically significant. There were significant post hoc differences between some of the target pairs for σy and BCEA, but not for σx. Monitor-based Bessel beam targets provided significantly smaller σy and BCEAs than the bull's eye/cross hair combination and the monitor- and laser-based Gaussian beam targets.
CONCLUSIONS Monitor-based images of Bessel beams provided better fixation targets than did a bull's eye/cross hair combination, monitor-based Gaussian images, and laser Gaussian beams, but no claim can be made that laser Bessel beams provide better fixation targets than do laser Gaussian beams. SIGNIFICANCE Noninvasive high-speed videokeratoscopy equipped with specific software has shown potential for assessing the homeostasis of tear film, providing clinicians with a fast and consistent tool for supporting dry eye diagnosis and management.
PURPOSE The purpose of this study was to evaluate the efficacy of a recently proposed method for characterizing tear film dynamics using noninvasive high-speed videokeratoscopy in assessing the loss of homeostasis of tear film.
METHODS Thirty subjects from a retrospective study, of which 11 were classified as dry eye and 19 as healthy, were included. High-speed videokeratoscopy measurements were performed using E300 videokeratoscope (Medmont Pty., Ltd., Melbourne, Australia). Raw data were analyzed using a recently proposed method to estimate the dynamics of the tear film based on a fractal dimension approach. This method provides three time-varying indicators related to the regularity of the reflected rings: tear film surface quality indicator, breaks feature indicator, and distortions feature indicator. From each indicator, five parameters were extracted and analyzed, including noninvasive breakup time, mean value of the indicator in the stability phase, mean value of the indicator in the whole interblink interval, mean value of the indicator in the leveling phase, and the general trend of the time series. Receiver operating characteristic curves were used to determine the sensitivity and specificity of each parameter in dry eye detection.
RESULTS The best discrimination performance between dry eye and healthy subjects was achieved with the breaks feature indicator noninvasive breakup time parameter, with an area under the curve of 0.85. For a cutoff value of 10 seconds, the sensitivity was 100% and the specificity was 84%.
CONCLUSIONS The analyzed method improves the assessment of tear film homeostasis in comparison with previous high-speed videokeratoscopy methods showing higher potential in assisting dry eye diagnosis.

Without appreciation of the optics involved, these effects might wrongly be considered artifacts.
PURPOSE The purpose of this study was to investigate the interactions of autorefractors with lenses and prisms.
METHODS There were 15 adult participants across three experiments, with a range of ages and refractions. In experiments 1 and 2, participants wore frames containing base-up and base-down nominally plano prisms. In experiment 3, participants wore a lens that produced either 6.3% magnification or 5.9% minification, depending on which surface faced the eye. Autorefracting instruments with different operating principles were used: Shin-Nippon SRW5000 autorefractor, Grand Seiko 5100K autorefractor, Hoya AR-530 autorefractor, a Complete Ophthalmic Analysis System–High Definition wavefront sensor, and Tomey FC-800 autorefractor. A theory on the likely effects of magnifying lenses was presented.
RESULTS For ophthalmic prisms, refractions showed results similar to those predicted on the basis of how effective prism powers change with pantoscopic tilt. As tilt increased, base-up prism gave more positive mean refractions and more negative horizontal/vertical astigmatism and vice versa for base-down prisms. In the presence of 10° tilt, 8Δ base-up prisms and 8Δ base-down prisms had different effects by a mean of 0.36 diopters. Magnifying lenses affected refractions similar to those predicted on the basis of vergence changes, with 6% magnification and minification producing mean changes of −11 and +8%, respectively, in absolute mean refraction. There was no strong evidence that different instruments had different effects.
CONCLUSIONS The results have implications for studies in which prisms and lenses are placed in the front eyes, such as accommodation studies using thick lenses close to the eyes to stimulate accommodation rather than by changing object distance.

Current Air Force evaluation of distortion is an entirely qualitative assessment of the acceptability of image distortion. The novel, quantitative technique described here is capable of identifying 0.1% distortion across an array of optical samples.
PURPOSE Optical distortion is the effect by which a transparent object spatially warps the perception of a visual target. All U.S. Air Force visors are required to pass military standards outlined in MIL-DTL-43511D (2006). Although specifications for the optical distortion setup and critical areas of vision are outlined, the evaluation technique is entirely qualitative, with a panel of several human evaluators assessing the distortion acceptability. The evaluation is not explicitly tied to a visual acceptability rating and has variable levels of consistency over time or across evaluators and a fabrication tolerance limit of 3% distortion.
METHODS The technique proposed in this article is a modification to the recommended optical tester used to analyze distortion patterns. An image-processing algorithm was developed to analyze patterns of Ronchi grid distortion mathematically to provide a quantitative approach that can subsequently be tied to visual metrics.
RESULTS This effort developed and refined an algorithm that allowed for a standardized assessment creation of high-resolution distortion maps from digital images. A 1-inch-diameter region imaged through ophthalmic material allowed for two-dimensional median filtering down to 15-pixel areas with enhanced contrast between grid lines leading to possible resolution capabilities of 0.10% distortion.
CONCLUSIONS Quantification of the standard for measuring optical distortion is the initial step toward determining the effects of distortions on human visual performance metrics. The future goal for this effort will focus on obtaining empirical results from human experimental efforts and relating the distortion location and magnitude to effects on visual performance activities.

[Thésaurus HELB]:Optique:pression intraoculaireRésumé : SIGNIFICANCE Precise measurement of intraocular pressure (IOP) is essential when diagnosing and managing glaucoma. We compared the IOP readings of three different tonometers and analyzed agreement among tonometers in eyes with high IOPs.
PURPOSE The purpose of this study was to compare the IOP readings obtained using a Goldmann applanation tonometer (GAT), a rebound tonometer (RT), and a Tono-Pen (TP) in angle-closure eyes with elevated IOP before and after medical IOP-lowering therapy.
METHODS Twenty-five eyes of 25 patients with angle closure and IOPs of greater than 30 mmHg were enrolled. Intraocular pressure was measured using RT (iCare Pro), TP (Tono-Pen XL), and GAT before and after medical treatment. The mean IOP readings of the tonometers were compared before and after treatment. The agreement among the tonometers was assessed via Bland-Altman analysis.
RESULTS The measurements from 22 eyes of 22 patients were suitable for statistical analyses. Before medical treatment, the mean TP-IOP was significantly lower than the mean GAT-IOP (44.0 ± 10.3 vs. 50.4 ± 8.9 mmHg, respectively; P < .001), but no significant difference was evident between the RT-IOP and the GAT-IOP (50.8 ± 10.9 vs. 50.4 ± 8.9 mmHg, respectively; P = .79). After IOP-lowering treatment, the mean GAT-IOP (14.9 ± 4.7 mmHg) did not differ from either the mean RT-IOP (15.6 ± 4.4 mmHg) or the mean TP-IOP (15.4 ± 5.0 mmHg; P = .05 and P = .18, respectively). The random measurement error among tonometers was greater for high IOP readings.
CONCLUSIONS Compared with RT or GAT, TP underestimated IOP in angle-closure eyes with a GAT-IOP of greater than 30 mmHg. Intraocular pressure reading agreement among the three tonometers was lower in eyes with high IOP.

[Thésaurus HELB]:Optique:lunettes télescopiquesRésumé : ABSTRACT This work challenges the standard of the past 40 years, which required the use of a bioptic telescope by individuals with vision loss wanting to be licensed to drive in most states in the United States.
Driving continues to be the key to independence for many individuals, particularly older drivers who live in an area where public transportation is limited or nonexistent. For the past 40 years, the most frequently option to allow drivers who are visually impaired to maintain driving privileges was to require them to use a bioptic telescope. Bioptic telescopes were felt to be necessary for wayfinding when driving. In addition, it was thought that a person could look through a bioptic telescope and still be aware of the driving environment around him/her. Human factor research has shown that the assertion that an individual can attend to two tasks simultaneously is not possible. Taking one's eyes off the road for as little as 2 seconds can lead to lane position breakdown. In 2018, wayfinding can now be more easily accomplished with the use of ubiquitous technologies like Global Positioning System systems on our telephones and in our cars. Driver distraction principles support safer alternatives to bioptic telescopes because these audio options allow the drivers to maintain their eyes and their attention on the road and the traffic around them. The switching of view within the bioptic spectacles is attentionally demanding, and the visual field restriction of such devices reduces overall situation awareness by narrowing the driver's attention.

[Thésaurus Mesh]ThérapeutiqueRésumé : SIGNIFICANCE Ophthalmic manifestations of non-Hodgkin lymphoma are rare, and the diagnosis can be delayed because of nonspecific symptoms and a tendency to mimic the appearance of other ocular diseases. Suspicious presentations will require confirmation of the lymphoma through surgical biopsy.
PURPOSE The purpose of this study was to present an isolated conjunctival non-Hodgkin marginal zone lymphoma without systemic involvement, which was successfully managed with external beam radiation.
CASE REPORT A 49-year-old man reported a red, temporal bulbar conjunctival growth on the left eye for several weeks, which was initially treated as a nodular episcleritis. When the condition did not resolve, a subsequent biopsy diagnosed a low-grade non-Hodgkin marginal zone lymphoma; further testing found no systemic involvement. No intraocular involvement was noted, and B-scan ultrasound and magnetic resonance imaging did not demonstrate any uveal or orbital extension. A trial of doxycycline did not lead to regression, and the isolated lesion was observed. Because of possible slow growth, the lesion was eventually treated with external beam radiation, which resulted in significant tumor regression. Four years after the radiotherapy, the tumor has not recurred, and there has been no systemic involvement.
CONCLUSIONS This case of a low-grade extranodal marginal zone lymphoma on the temporal conjunctiva represents a less common variation of the classic fornix-based presentation. It is not unusual for conjunctival lymphomas to masquerade as another clinical entity, sometimes making the initial diagnosis challenging. If a patient does not respond as expected to conventional therapy, a biopsy to rule out malignancy should be considered.

Long-term follow-up and contact lens fitting after penetrating keratoplasty provide best possible outcomes.
PURPOSE The purpose of this study was to report a case of LCAT deficiency successfully treated with penetrating keratoplasty and longer-term follow-up with contact lens fitting.
CASE REPORT A 43-year-old white woman of Italian descent presented with corneal clouding and trouble with night vision. The patient had a history of LCAT deficiency, irritable bowel syndrome, gastroesophageal reflux disease, osteoporosis, and hemolytic anemia. Slit-lamp examination demonstrated corneal haze throughout the corneal layers. The corneas had normal pachymetry. Given the opacity of each cornea (right greater than left) and decreased night vision, penetrating keratoplasty was performed on the right eye. At post-operative month 16, the corneal graft remained clear. The patient was able to achieve a best-corrected visual acuity of 20/30+ with a scleral lens.
CONCLUSIONS Penetrating keratoplasty may be necessary to provide better quality of vision in LCAT deficiency patients, specifically to enhance one's contrast sensitivity, despite relatively good Snellen visual acuity.Permalink : https://bibliotheque.helb-prigogine.be/opac_css/index.php?lvl=notice_display&id= [article]Exemplaires
