Lymphome malin non hodgkinien
Any of a group of malignant tumors of lymphoid tissue that differ from HODGKIN DISEASE, being more heterogeneous with respect to malignant cell lineage, clinical course, prognosis, and therapy. The only common feature among these tumors is the absence of giant REED-STERNBERG CELLS, a characteristic of Hodgkin's disease.

[Thésaurus Mesh]ThérapeutiqueRésumé : SIGNIFICANCE Ophthalmic manifestations of non-Hodgkin lymphoma are rare, and the diagnosis can be delayed because of nonspecific symptoms and a tendency to mimic the appearance of other ocular diseases. Suspicious presentations will require confirmation of the lymphoma through surgical biopsy.
PURPOSE The purpose of this study was to present an isolated conjunctival non-Hodgkin marginal zone lymphoma without systemic involvement, which was successfully managed with external beam radiation.
CASE REPORT A 49-year-old man reported a red, temporal bulbar conjunctival growth on the left eye for several weeks, which was initially treated as a nodular episcleritis. When the condition did not resolve, a subsequent biopsy diagnosed a low-grade non-Hodgkin marginal zone lymphoma; further testing found no systemic involvement. No intraocular involvement was noted, and B-scan ultrasound and magnetic resonance imaging did not demonstrate any uveal or orbital extension. A trial of doxycycline did not lead to regression, and the isolated lesion was observed. Because of possible slow growth, the lesion was eventually treated with external beam radiation, which resulted in significant tumor regression. Four years after the radiotherapy, the tumor has not recurred, and there has been no systemic involvement.
CONCLUSIONS This case of a low-grade extranodal marginal zone lymphoma on the temporal conjunctiva represents a less common variation of the classic fornix-based presentation. It is not unusual for conjunctival lymphomas to masquerade as another clinical entity, sometimes making the initial diagnosis challenging. If a patient does not respond as expected to conventional therapy, a biopsy to rule out malignancy should be considered.

[Thésaurus Mesh]Lymphome malin non hodgkinienRésumé : L’International Network on Cancer, Infertility and Pregnancy (INCIP) recense les
femmes chez qui un cancer a été diagnostiqué pendant la grossesse, afin de mieux comprendre
les résultats obstétricaux et oncologiques. L’INCIP a récemment publié dans le British
Journal of Haematology une série de 80 femmes enceintes chez qui un lymphome non hodgkinien
a été diagnostiqué entre 1986 et 2019. Le sous-type le plus fréquent était le lymphome
diffus à grandes cellules B (LDGCB) (57,71%), et 54 femmes (68%) ont été traitées par chimiothérapie,
principalement de type CHOP (cyclophosphamide, doxorubicine, vincristine, prednisone)
(89%), durant leur grossesse. Quatre femmes ont opté pour une interruption prématurée
de leur grossesse. Sur les 76 grossesses qui ont été poursuivies, une seule s’est soldée
par un enfant mort-né (1,3%). L’incidence du faible poids de naissance (39%), de l’accouchement
avant terme (52%), ainsi que des complications obstétricales (41%) et néonatales (12%)
s’est avérée globalement élevée. Ces chiffres ne peuvent pas uniquement s’expliquer par la
chimiothérapie prénatale. La moitié des accouchements avant terme (46%) ont été programmés
pour des raisons d’ordre oncologique. La survie globale et la survie sans progression
à 3 ans des patientes atteintes d’un LDGCB traitées par rituximab-CHOP s’élevaient respectivement
à 95,7% et 83,4% pour le stade limité (stade I-II selon la classification d’Ann Arbor)
(n =29), ainsi qu’à 73,3% et 60,6% pour le stade avancé (stade III-IV selon la classification
d’Ann Arbor) (n=15). Parmi les 36 patientes enceintes qui ont reçu du rituximab, 5 cas (13%)
de complications néonatales et 3 infections maternelles (8%) ont été rapportés. En conclusion,
un traitement standard pour le LDGCB peut être proposé aux patientes enceintes dans un
