[Thésaurus Mesh]Instabilité articulaireRésumé : Cette fiche est une proposition d’exercices pratiques pouvant permettre d’améliorer la prise en charge kinésithérapique des instabilités de l’épaule. Elle n’a pas vocation à être exhaustive mais de proposer un complément à votre arsenal thérapeutique.
L’épaule est une articulation très sollicitée par les gestes de la vie quotidienne et la pratique sportive. L’épaule doit répondre à 4 particularités pour être fonctionnelle : la mobilité, la force, la vitesse et la stabilité. Cependant, de par sa configuration, l’épaule est plus orientée vers la mobilité que la stabilité la rendant plus vulnérable à l’instabilité. Lors de la pratique sportive, l’épaule peut être sollicitée dans des amplitudes extrêmes, subir des traumatismes directs ou des microtraumatismes répétés. La pratique sportive est responsable de la moitié des instabilités traumatiques. [Thésaurus Mesh]Prothèse articulaireRésumé : Parmi les prothèses d’épaule, nous retrouvons classiquement les prothèses humérales simples, les prothèses totales anatomiques et les prothèses totales inversées. Ces dernières sont des prothèses semi-contraintes.
• La prothèse inversée a été inventée par Paul-Marie Grammont, chirurgien orthopédiste Dijonnais, suite à un constat unanime réalisé dans les années 1970 : « Les résultats fonctionnels obtenus après arthroplastie prothétique d’épaule dans les omarthroses associées à une rupture irréparable de la coiffe des rotateurs sont insatisfaisants. ».La première prothèse inversée dite de Grammont a été posée 16 ans plus tard en 1986. La diffusion de ce type de prothèse devient mondiale à partir du début des années 2000.
• L’objectif des prothèses d’épaule inversée est d’obtenir une élévation de l’épaule grâce exclusivement au deltoïde.
La récupération de la rotation latérale active reste sous la dépendance de l’intégrité des muscles infra-épineux, petit rond et du deltoïde postérieur.Cette prothèse inversée est composée (fig. 1) du côté huméral d’une cupule orientée avec une inclinaison de 155°, du côté glénoïdien d’une large hémisphère sans col placée directement sur la cupule glénoïdienne. Le centre de rotation est ainsi déplacé sur la cupule glénoïdienne et l’abaissement relatif obtenu de l’humérus par rapport à l’acromion permet d’obtenir une mise en tension des fibres du deltoïde et une efficacité accrue.Permalink : https://bibliotheque.helb-prigogine.be/opac_css/index.php?lvl=notice_display&id= [article]Exemplaires
