[Thésaurus Mesh]MyopieRésumé : SIGNIFICANCE Results of this randomized, double-masked clinical trial demonstrate the effectiveness of the MiSight soft contact lens in slowing myopia progression over multiple years.
PURPOSE The purpose of this study was to quantify the effectiveness of MiSight daily disposable soft contact lens in slowing the progression of juvenile-onset myopia.
METHODS Myopic children (spherical equivalent refraction, −0.75 to −4.00 D; astigmatism, <1.00 D) aged 8 to 12 years with no prior contact lens experience were enrolled in a 3-year, double-masked, randomized clinical trial at four investigational sites in four countries. Subjects in each group were matched for age, sex, and ethnicity and were randomized to either a MiSight 1-day contact lens (test) or Proclear 1-day (control; omafilcon A) and worn on a daily disposable basis. Primary outcome measures were the change in cycloplegic spherical equivalent refraction and axial length.
RESULTS Of the subjects enrolled, 75.5% (109/144) completed the clinical trial (53 test, 56 control). Unadjusted change in spherical equivalent refraction was −0.73 D (59%) less in the test group than in the control group (−0.51 ± 0.64 vs. –1.24 ± 0.61 D, P < .001). Mean change in axial length was 0.32 mm (52%) less in the test group than in the control group (0.30 ± 0.27 vs. 0.62 ± 0.30 mm, P < .001). Changes in spherical equivalent refraction and axial length were highly correlated (r = −0.90, P < .001). Over the course of the study, there were no cases of serious ocular adverse events reported. Four asymptomatic corneal infiltrative (one test, three control) events were observed at scheduled study visits.

[Thésaurus Mesh]Relations médecin-patientRésumé : En oncologie, les programmes de formation à la communication ont montré une efficacité sur les compétences en communication des médecins. Cette étude évalue l’impact d’un tel programme dans un contexte d’essais thérapeutiques de phase I. Une évaluation de la satisfaction et de l’efficacité personnelle perçue concernant la communication médecin–patient était réalisée chez 6 médecins (3 juniors et 3 séniors) avant et après une formation à la communication médicale. Soixante entretiens ont été réalisés. L’étude, non randomisée, distinguait les consultations oncologue et patient seul (situation « duelle ») des consultations en présence d’un proche (situation « triangulaire »). La comparaison des scores d’efficacité personnelle avant et après formation montre une progression des scores pour l’ensemble des médecins, en situation duelle comme en situation triangulaire. Cette progression touche essentiellement les médecins juniors dans les situations de consultation triangulaire. De même, les scores de satisfaction montrent une satisfaction avant formation moindre en situation triangulaire que duelle (p=0,01), cela étant plus marqué pour les médecins juniors que pour les médecins séniors (p=0,035). À l’issue de la formation, en situation triangulaire, le score des médecins juniors rejoint celui des séniors. La formation à la communication bénéficie particulièrement aux médecins juniors en situation triangulaire. To determine the added benefit of combining dry needling with a guideline-based physical therapy treatment program consisting of exercise and manual therapy on pain and disability in people with chronic neck pain.
Design: Randomized controlled trial.
Methods
Participants were randomized to receive either guideline-based physical therapy or guideline-based physical therapy plus dry needling. The primary outcomes, measured at 1 month post randomization, were average pain intensity in the previous 24 hours and previous week, measured with a numeric pain-rating scale (0–10), and disability, measured with the Neck Disability Index (0–100). The secondary outcomes were pain and disability measured at 3 and 6 months post randomization and global perceived effect, quality of sleep, pain catastrophizing, and self-efficacy measured at 1, 3, and 6 months post randomization.
Results
One hundred sixteen participants were recruited. At 1 month post randomization, people who received guideline-based physical therapy plus dry needling had a small reduction in average pain intensity in the previous 24 hours (mean difference, 1.56 points; 95% confidence interval [CI]: 1.11, 2.36) and in the previous week (mean difference, 1.20 points; 95% CI: 1.02, 2.21). There was no effect of adding dry needling to guideline-based physical therapy on disability at 1 month post randomization (mean difference, −2.08 points; 95% CI: −3.01, 5.07). There was no effect for any of the secondary outcomes.
Conclusion
When combined with guideline-based physical therapy for neck pain, dry needling resulted in small improvements in pain only at 1 month post randomization. [Thésaurus Mesh]Troubles de la visionRésumé : SIGNIFICANCE The results of this study suggest that clinicians providing vergence/accommodative therapy for the treatment of childhood convergence insufficiency should not suggest that such treatment, on average, will lead to improvements on standardized assessments of reading performance after 16 weeks of treatment.
PURPOSE The purpose of this study was to determine the effect of office-based vergence/accommodative therapy on reading performance in 9- to 14-year-old children with symptomatic convergence insufficiency.
METHODS In a multicenter clinical trial, 310 children 9 to 14 years old with symptomatic convergence insufficiency were randomized in a 2:1 ratio to 16 weeks of office-based vergence/accommodative therapy or office-based placebo therapy, respectively. The primary outcome was change in reading comprehension as measured by the reading comprehension subtest of the Wechsler Individual Achievement Test, Third Edition (WIAT-III) at the 16-week outcome. Secondary reading outcomes of word identification, reading fluency, listening comprehension, comprehension of extended text, and reading comprehension were also evaluated.
RESULTS The adjusted mean improvement in WIAT-III reading comprehension was 3.7 (95% confidence interval [CI], 2.6 to 4.7) standard score points in the vergence/accommodative therapy group and 3.8 (95% CI, 2.4 to 5.2) points in the placebo therapy group, with an adjusted mean group difference of −0.12 (95% CI, −1.89 to 1.66) points that was not statistically significant. No statistically significant treatment group differences were found for any of the secondary reading outcome measures.
CONCLUSIONS For children aged 9 to 14 years with symptomatic convergence insufficiency, office-based vergence/accommodative therapy was no more effective than office-based placebo therapy for improving reading performance on standardized reading tests after 16 weeks of treatment.
