will provide good functional outcomes which are maintained for over 5 years.
Method: Prospective study of 12 consecutive cases with a minimal 5-year follow-up were recruited from April
2008 to November 2010. Cases whom were mentally incompetent or those which required concomitant procedures
were excluded. Outcomes measures included the numeric pain rating scale, motor power strength, range
of motion, functional scoring using the AOFAS hindfoot score and patient’s self-assessment using the Foot and
Ankle Outcome Score (FAOS).
Result: No re-ruptures or sural nerve injured were identified after a minimal 5-year follow-up. Pain was minimal
at 0.5/10, calf power was 5/5 and ankle range was good (plantarflexion: 38°/dorsiflexion: 21°). The AOFAS
hindfoot score was 97.4 and all 5 sub-categories of the Foot and Ankle Outcome Score (FAOS) were good.
Conclusion: The 1.5 cm medial incision repair of the Achilles tendon is an economically sound surgical technique,
with minimal complications, which gives good medium length functional outcomes.

[Thésaurus Mesh]Tendon calcanéenRésumé : La tendinopathie Achilléenne est une pathologie musculo-squelettique fréquente, à l’origine de douleurs localisées au tendon d’Achille et d’une impotence fonctionnelle. À ce jour, environ un patient sur trois ne répond pas au traitement par les exercices de renforcement, qui est le traitement de première intention d’après les recommandations de bonnes pratiques pour la rééducation de la tendinopathie d’Achille. Notre rééducation de la tendinopathie Achilléenne n’est donc pas optimale, probablement car nous n’avons pas encore entièrement compris les mécanismes sous-jacents à la pathologie. Notre approche en rééducation est principalement basée sur la quantification de la contrainte appliquée au tendon, ce dernier étant alors considéré comme une entité unique. Cependant, le tendon d’Achille est compartimenté en trois sous-tendons. Nous savons aujourd’hui que son comportement biomécanique n’est pas uniforme chez le sujet sain, et qu’il est altéré en présence d’une tendinopathie Achilléenne. Cela suggère que la distribution des contraintes mécaniques subies par le tendon d’Achille, et non seulement l’amplitude des contraintes, joue un rôle dans la physiopathologie de la tendinopathie Achilléenne. Plusieurs facteurs sont probablement impliqués : anatomie, propriétés biomécaniques intrinsèques des tissus, contrôle moteur des muscles attachés au tendon d’Achille. In the presence of a large gap where end-to-end repair of the torn Achilles tendon is difficult and V-Y advancement would likely be insufficient, augmentation is sometimes required. At our institute we have used primarily the hamstring autograft augmentation technique for the past two decades. The aim of this study was to analyze the complications after surgical treatment of Achilles tendon rupture with semitendinous tendon augmentation.
Methods
We retrospectively analyzed 58 consecutive patients treated with semitendinous tendon autograft augmentation at the Helsinki University Hospital between January 1, 2006, and January 1, 2016.
Results
During the study period, 58 patients were operated on by six different surgeons. Of 14 observed complications (24%), seven were major and seven were minor. Most of the complications were infections (n = 10 [71%]) The infections were noted within a mean of 62 days postoperatively (range, 22–180 days). Seven patients with a complication underwent repeated operation because of skin edge necrosis and deep infection (five patients), hematoma formation (one patient), and a repeated rupture (one patient).
Conclusions
In light of the experience we have had with autologous semitendinous tendon graft augmentation, we cannot recommend this technique, and, hence, we should abandon reconstruction of Achilles tendon ruptures with autologous semitendinous tendon grafts at our institute.
