[Thésaurus Mesh]OrthopédieRésumé : Les thérapies manuelles ont une longue histoire remontant à l’Antiquité, au XIXe siècle, elles ont regagné en popularité avec le développement de la physiothérapie. Au milieu du XXe siècle, la thérapie manuelle s’est organisée au niveau international avec la création de l’IFOMPT, établissant des normes d’expertise neuro-musculo-squelettique (NMS). Aujourd’hui, l’IFOMPT vise à promouvoir cette expertise en NMS et à diversifier ses membres pour améliorer la prise en charge des patients dans ce domaine. Il est essentiel de prêter attention à la sémantique des mots et à leur interprétation. Chacun des termes doit être utilisés de manière appropriée afin d’éviter les erreurs et confusions dans les réflexions et interprétations de données. L’expression de « Techniques Manuelles » ne devrait être utilisée que pour définir les techniques utilisant les mains, et ne pas inclure les processus cognitifs gravitant autour. De par son ambiguïté sémantique et phonologique, la locution de « Thérapie Manuelle » devrait être éviter sauf pour décrire les approches et théories ayant participé à l’émergence de la Thérapie Manuelle Orthopédique. La finalité de la TMO est de soigner le patient présentant un trouble neuro-musculo-squelettique. Cela passe par la mise en place d’un traitement spécifique à la problématique unique du patient. Le traitement spécifique en TMO est le résultat de la prise de décision partagée entre le kinésithérapeute et le patient. Le kinésithérapeute pratiquant la TMO est guidé par les preuves disponible afin d’apporter à travers son raisonnement : son expertise et ses compétences (cognitives et techniques) en neuro-musculo-squelettique (NMS) sous couvert d’être influencé par ses biais cognitifs et ses préférences. Dans le cadre d’un traitement en TMO, le patient représente une problématique clinique unique à travers la prise en compte des facteurs biopsychosociaux (BPS) et de ses préférences. [Thésaurus Mesh]Revue de la littératureRésumé : OBJECTIVE: We aimed to estimate the benefits and harms of cervical spinal manipulative therapy (SMT) for treating neck pain.
DESIGN: Intervention systematic review with meta-analysis of randomized controlled trials (RCTs).
LITERATURE SEARCH: We searched the MEDLINE, Cochrane CENTRAL, Embase, CINAHL, PEDro, Chiropractic Literature Index bibliographic databases, and grey literature sources, up to June 6, 2022.
STUDY SELECTION CRITERIA: RCTs evaluating SMT compared to guideline-recommended and nonrecommended interventions, sham SMT, and no intervention for adults with neck pain were eligible for our systematic review. Prespecified outcomes included pain, range of motion, disability, health-related quality of life.
DATA SYNTHESIS: Random-effects meta-analysis for clinically homogenous RCTs at short-term and long-term outcomes. Risk of bias was assessed using the Cochrane Risk of Bias 2.0 Tool. We used the Grading of Recommendations, Assessment, Development, and Evaluations approach to judge the certainty of evidence.
RESULTS: We included 28 RCTs. There was very low to low certainty evidence that SMT was more effective than recommended interventions for improving pain at short term (standardized mean difference [SMD], 0.66; 95% confidence interval [CI]: 0.35, 0.97) and long term (SMD, 0.73; 95% CI: 0.31, 1.16), and for reducing disability at short-term (SMD, 0.95; 95% CI: 0.48, 1.42) and long term (SMD, 0.65; 95% CI: 0.23, 1.06). Transient side effects only were found (eg, muscle soreness).

Objective
To examine the use of traction by physical therapists for patients with neck pain, including how and when traction is used, the delivery modes and parameters, supplemental interventions, and influence of professional demographics on usage.
Background
Systematic reviews provide little support for traction in managing neck pain. While traction usage among physical therapists in other countries has been described, usage in the United States has not been examined.
Methods
A random sample of 4000 Orthopaedic Section members of the American Physical Therapy Association were surveyed. Associations among respondents' professional demographics and survey responses were explored with chi-square analyses (α = .05).
Results
The response rate was 25.5% (n = 1001); 76.6% (n = 767) of respondents reported using traction, and 93.1% reported that they would use traction for a patient presenting with signs of nerve root compression. Common delivery modes included manual methods (92.3%) and mechanical traction tables (88.3%). Using traction is often supplemented with other interventions (exercise, postural education, joint mobilization). A higher proportion of physical therapists with American Board of Physical Therapy Specialties orthopaedic certification reported using traction (88.6%) than did those without specialty certification (73.0%).
Conclusion
Most respondents reported that their use of cervical traction was consistent with proposed criteria that identify patients who are likely to benefit. They use various traction delivery modes/parameters within comprehensive plans of care incorporating multiple interventions.
