Background
Individuals with low back pain have impaired activation of the multifidus during postural adjustments and increased activity of the erector spinae musculature during walking. However, it is unclear whether these alterations in muscle activity are evident during locomotion in individuals with a history of low back pain when they are between symptomatic episodes.
Objectives
To compare paraspinal muscle activity in young, healthy individuals and young individuals with a history of low back pain during walking turns.
Methods
Fourteen asymptomatic individuals with a history of low back pain and 14 controls performed 90° walking turns at both a self-selected speed and a fast speed. The duration and amplitude of activity in the deep fibers of the multifidus and the lumbar and thoracic longissimus were quantified using intramuscular electromyography.
Results
There was a significant speed-by-group interaction for the duration of multifidus activity (P = .013). Duration of activity increased from the self-selected speed to the fast locomotor speed in the controls, but decreased in the individuals with a history of low back pain (P = .003). Self-selected speed was the same in both groups (P = .719). There was a trend toward a significant association between group and the direction of change in the duration of deep multifidus activity (χ2 = 0.058). Duration of thoracic longissimus activity and amplitude of multifidus and thoracic longissimus activity increased similarly in both groups from the self-selected to the faster speed.
Conclusion
Even between symptomatic episodes, young individuals with a history of low back pain demonstrated altered recruitment of the deep fibers of the lumbar multifidus in response to changing locomotor speed during walking turns.

OBJECTIFS: Évaluer la dynamique de l'articulation du genou chez des joueurs d'élite de volleyball avec et sans antécédents de tendinopathie rotulienne, en mettant l'accent sur l'absorption et la génération d'énergie mécanique. Nous avons présumé que la tendinopathie serait associée à une réduction de l'activité et de la puissance nettes de l'articulation INFORMATIONS GÉNÉRALES: La tendinopathie rotulienne est une pathologie courante et débilitante qui affecte les joueurs de volleyball de compétition. MÉTHODES: Treize joueurs d'élite avec et sans antécédents de tendinopathie rotulienne (âge + DS moyen, 27 + 7 ans) ont effectué des sauts d'approche au volleyball avec effort maximum. Les données cinématiques, cinétiques et énergétiques du plan sagittal de l'articulation du genou ont été quantifiées dans le membre principal, en utilisant les données recueillies au moyen d'une plate-forme de force et d'un système d'analyse de mouvement comprenant 8 caméras. Les forces verticales de réaction au sol et la vélocité verticale du pelvis au départ ont été examinées. Des tests sur échantillons indépendants ont été utilisés pour évaluer les différences entre les groupes (a = 0,05). RÉSULTATS: Lorsque comparé au groupe de contrôle, le groupe souffrant de tendinopathie a présenté des réductions significatives (environ 30 %) de l'activité et de la puissance nettes de l'articulation durant la phase excentrique du saut, mais aucune différence durant la phase concentrique. Les ratios positif à négatif d'activité et de puissance nettes de l'articulation étaient nettement supérieurs dans le groupe souffrant de tendinopathie, qui avait un ratio d'activité nette d'articulation de 1,00 (IC 95 % : 0,77, 1,24) contre 0,76 (IC 95 % : 0,64, 0,88) pour le groupe de contrôle et un ratio de puissance nette de l'articulation de 1,62 (IC 95 % : 1,15, 2,10) contre 1,00 (IC 95 % : 0,80, 1,21) pour le groupe de contrôle. Aucune différence significative n'est apparue dans le moment articulaire net, la vélocité angulaire ou l'amplitude de mouvement. Les forces de pointe de réaction verticale au sol étaient inférieures chez le groupe souffrant de tendinopathie, tandis que les forces moyennes de réaction verticale au sol et la vélocité verticale du pelvis chez le groupe souffrant de tendinopathie étaient similaires aux forces moyennes de réaction verticale au sol et à la vélocité verticale du pelvis chez le groupe de contrôle. CONCLUSION: La tendinopathie rotulienne est associée à des différences dans l'absorption d'énergie mécanique du plan sagittal au niveau du genou durant les sauts d'approche au volleyball avec effort maximum. [Thésaurus Mesh]TroncRésumé : Background Motor control dysfunction persisting during symptom remission in persons with recurrent low back pain (LBP) may contribute to the recurrence of pain. Objectives To investigate trunk control in persons in remission from recurrent LBP and in back-healthy controls using a dynamic, internally driven balance task. No differences in task performance were expected between groups, but it was hypothesized that persons with recurrent LBP would exhibit greater trunk coupling, consistent with a trunk-stiffening strategy. Methods In this cross-sectional controlled laboratory study, persons with and without recurrent LBP (n = 19 per group) completed the balance-dexterity task, which involved balancing on one limb in standing while compressing an unstable spring with the other. Task performance measures included center-of-pressure velocity under the stance limb and vertical force variability under the spring. Trunk coupling was quantified with the coefficient of determination (R2) of an angle-angle plot of thorax-pelvis frontal plane motion. Fine-wire and surface electromyography captured activations of paraspinals and abdominals. Results There were no differences between groups for any task performance measure. The group in remission from recurrent LBP exhibited reduced trunk coupling, or more dissociated thorax and pelvis motion, compared to the healthy control group (P = .024). Trunk coupling in this group was associated moderately with the lumbar multifidus-to-erector spinae activation ratio (r = 0.618, P = .006) and weakly with the internal oblique-to-external oblique ratio (r = 0.476, P = .046). Conclusion The balance-dexterity task is a submaximal, internally driven unstable balance task during which more dissociated trunk motion was observed in persons in remission from recurrent LBP. Findings underscore the task-dependent nature of trunk control research and assessment in persons with recurrent LBP.
