[Thésaurus Mesh]ThérapeutiqueRésumé : Objectifs : Actualiser sous l’égide de la section os de la Société française de rhumatologie (SFR) et du groupe de recherche et d’information sur les ostéoporoses (GRIO) en collaboration avec des sociétés savantes (Collège national des généralistes enseignants, Collège national des gynécologues et obstétriciens français, Fédération nationale des collèges de gynécologie médicale, groupe d’étude de la ménopause et du vieillissement hormonal, Société française de chirurgie orthopédique, Société française d’endocrinologie, Société française de gériatrie et de gérontologie) les recommandations du traitement médicamenteux de l’ostéoporose post-ménopausique publiées précédemment en 2012. Méthodes : Un groupe de travail, représentatif des spécialités médicales intervenant dans la prise en charge de ces patientes a élaboré ces recommandations à partir d’une analyse systématique de la littérature selon la méthode HAS. Discussion et conclusion : Ces recommandations insistent sur la prise en charge des femmes avec une fracture sévère chez lesquelles un traitement anti-ostéoporotique est recommandé. En cas de fracture sévère, tous les traitements peuvent être prescrits ; l’acide zolédronique est à privilégier en première intention après une fracture de hanche. Dans les autres cas (avec ou sans fracture non sévère) l’indication thérapeutique dépend des valeurs de la densité minérale osseuse (DMO) et dans les cas difficiles d’outils comme le FRAX®. Tous les traitements peuvent être utilisés ; le raloxifène est à réserver aux patientes à faible risque de fracture périphérique. Le risque de fracture doit être réévalué tous les 2 à 3 ans pour décider des suites de la prise en charge. [Thésaurus HELB]:Optique:port de lentilles de contactRésumé : SIGNIFICANCE: Soft contact lens wear may lead to physiological changes in the cornea. Corneal densitometry is a new software tool that may detect these physiological changes by monitoring their effects on corneal transparency. PURPOSE: This study aimed to assess corneal densitometry values and corneal endothelium in patients using soft contact lenses. METHODS: The study included 36 eyes of 36 subjects with soft contact lens use (group 1) and 36 eyes of 36 healthy control subjects (group 2). Anterior segment parameters and corneal densitometry of all subjects were evaluated using Scheimpflug corneal topography (Pentacam; Oculus Inc., Wetzlar, Germany). A total corneal area of 12 mm was divided into four annular zones (0 to 2, 2 to 6, 6 to 10, and 10 to 12 mm) and into three regions as anterior, central, and posterior for densitometry examination. Cell density, cell distribution, and cell morphology (quadrangular, pentagonal, hexagonal, and other cell percentages) of the endothelial layer were examined in both groups using specular microscopy (Nidek CEM 530; Nidek Co., Gamagori, Aichi, Japan). RESULTS: In group 1, the mean duration of contact lens wear was 4.45 ± 2.15 years. The anterior segment parameters were not significantly different between the two groups (P > .05). The anterior 0- to 2- and 2- to 6-mm corneal densitometry values were significantly higher in group 1 compared with group 2 (23.2 ± 4.7 for group 1 and 17.3 ± 5.6 for group 2 [P = .03]; 22.4 ± 6.4 for group 1 and 17.8 ± 1.4 for group 2 [P = .03], respectively). There were no significant differences between the groups for corneal densitometry values between 6 and 12 mm, or for cell density, cell distribution, and cell morphology (P > .05). CONCLUSIONS: Although the cornea remains transparent in soft contact lens wearers, corneal densitometry may increase in the 0- to 6-mm annular zone of the anterior region. The severe acute respiratory syndrome coronavirus 2 virus, which causes the coronavirus disease 2019 (COVID-19) pandemic, can bind to epithelial cells in the human cornea and conjunctiva, which may result in changes of corneal and lenticular densitometry.
PURPOSE
We aimed to report the corneal and lenticular clarity of patients who had been diagnosed previously with confirmed infection of COVID-19.
METHODS
This was a prospective, cross-sectional study. Fifty-three patients who had recovered from COVID-19 and 51 healthy individuals who had not had COVID-19 (control) were included in this study. Measurement of the corneal (at a corneal diameter of 12 mm) and lens densitometry of participants was performed using a Pentacam HR Scheimpflug imaging system. Three different optimal depths as anterior, central, and posterior layers and four concentric zones, consisting of diameters of 0 to 2, 2 to 6, 6 to 10, and 10 to 12 mm, were chosen to perform the corneal densitometric measurements. The mean lenticular densitometric values were calculated in zone 1 at 2.0 mm, zone 2 at 4.0 mm, and zone 3 at 6.0 mm by taking the central part of the pupil as reference point.
RESULTS
When compared with the individuals in the control group, the corneal densitometry measurements in the patients in the COVID-19 group were considerably higher in anterior 0 to 2 mm, 2 to 6 mm, 6 to 10 mm zones and total diameter; center 0 to 2 and 2 to 6 mm zones and total diameter; posterior 0 to 2 mm zone; and total corneal 0 to 2 and 2 to 6 mm zones and total diameter (P < .05, for each). When compared with the individuals in the control group, all of the lens densitometry measurements, except for those located in zone 1, were determined to be significantly higher among the patients in the COVID-19 group (P < .05, for each).
CONCLUSIONS
