Acute posterior multifocal placoid pigment epitheliopathy is an uncommon inflammatory chorioretinopathy that has been reported after vaccination. This is the first reported case, to our knowledge, after vaccination for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in a healthy adolescent boy.
PURPOSE
This report aimed to inform the eye care community about a possible ocular sequela of SARS-CoV-2 vaccination.
CASE REPORT
A 17-year-old boy presented to a clinic for a second opinion after sudden-onset blind spots in his right eye. His medical history was remarkable for receiving the first dose of the Pfizer-BioNTech SARS-CoV-2 vaccine 2 weeks before symptom onset. He had no history of ocular inflammation, autoimmune disease, or systemic infection. A diagnosis of unilateral acute posterior multifocal placoid pigment epitheliopathy was made based on the presence of typical fundus lesions and noninvasive imaging with fundus autofluorescence, retinal optical coherence tomography, and optical coherence tomography angiography. The diagnosis was further confirmed with fluorescein angiography. The patient developed an anterior vitritis in the right eye 42 days after initial symptom onset. His unilateral intraocular inflammation resolved after a 5-week course of prednisone.
CONCLUSIONS
Acute posterior multifocal placoid pigment epitheliopathy is a self-limited inflammatory condition of the outer retina that usually affects young adults and often does not require treatment. It has been reported to occur after vaccination for influenza, polio, hepatitis B, meningococcus C, and varicella zoster virus. This is the first known case to occur after SARS-CoV-2 vaccination in a healthy adolescent boy.Permalink : https://bibliotheque.helb-prigogine.be/opac_css/index.php?lvl=notice_display&id= [article]Exemplaires

[Thésaurus Mesh]Solutions ophtalmiquesMots-clés : netarsudil hémorragies intracornéennes Résumé : SIGNIFICANCE: Intracorneal hemorrhages are a rare finding generally associated with surgery or trauma. There is
no consensus on preferred management except eliminating or addressing the causative mechanism in hopes of reducing the risk of corneal haze or scarring.
PURPOSE: This case highlights a rare adverse outcome of intracorneal hemorrhages occurring after recent initiation
of netarsudil, possibly exacerbated by scleral contact lens wear in a patient with open-angle glaucoma and limbal
stem cell deficiency.
CASE REPORT: A 77-year-old man using scleral contact lenses for therapeutic management of limbal stem cell
deficiency started netarsudil for open-angle glaucoma. During an annual follow-up to adjust his scleral contact
lenses, the patient developed peripheral intracorneal hemorrhages bilaterally. The intracorneal hemorrhages resolved over the course of 10 weeks after minor adjustments were made to the scleral contact lens fit and netasurdil
was discontinued. Visual acuity and intraocular pressure remained stable throughout.
CONCLUSIONS: There are few reports of intracorneal hemorrhages associated with scleral contact lens use and
even fewer associated with the use of netarsudil. This case proposes several possible causes of the intracorneal
hemorrhages, including topical rho-associated kinase inhibitors, contact lens wear, and trauma. Further studies are
needed to determine if netarsudil is associated with intracorneal hemorrhages, to understand the sequelae of
intracorneal hemorrhages in netarsudil therapy, and to recommend management when intracorneal hemorrhages
manifest with netarsudil usePermalink : https://bibliotheque.helb-prigogine.be/opac_css/index.php?lvl=notice_display&id= [article]Exemplaires

[Thésaurus Type de publication]Présentations de casRésumé : SIGNIFICANCE: Vaccine-associated uveitis has appeared in recent years because of various vaccines, whereas cases for human papillomavirus (HPV) vaccination were rarely reported. With more and more females becoming aware of its importance and choosing HPV vaccination, much more attention should be paid on the adverse effects of it. PURPOSE: The purpose of this study was to report a rare case of posterior uveitis after divalent HPV vaccination in an Asian female. CASE REPORT: A 29-year-old woman presented with acute vision loss accompanied by symptoms of headache, tinnitus, and myalgia after the third injection of HPV vaccination. The best-corrected visual acuity dropped to 20/500 for both eyes, and optical coherence tomography revealed bilateral multifocal submacular fluid. A short course of oral prednisone as well as Ozurdex intravitreal injection resulted in the reversal of all signs and symptoms. CONCLUSIONS: Although this case resembled Harada disease, we diagnosed it as vaccine-induced uveitis rather than coincidental autoimmune disease based on the rapid response to a short course of systemic corticosteroids. Because vaccine-induced uveitis is rare and difficult to distinguish from coincidental autoimmune disease, our case reminds eye care providers to be aware of the possible association between vaccination and a Harada-like reaction and to ask appropriately directed questions when obtaining history from young patients with uveitis. Based on this case, we also suggest Ozurdex intravitreal injection as a potential therapeutic choice, especially for patients with contraindication or personal concern to systemic corticosteroid.
