Both consumers and eye care practitioners need to know how well online optical vendors conform with the accepted standards for quality and safety. We found that almost 1 in 10 prescriptions failed to meet national standards, which is a significant improvement over previous studies.
PURPOSE
The purpose of this study was to assess the accuracy and repeatability of spectacle lens prescriptions ordered from a sample of online vendors.
METHODS
Spectacle lens prescriptions were ordered by paid participants with no optical training who were masked to the study objectives. The prescription powers ordered (sphere, cylinder, and axis) were statistically sampled from 1000 previously filled prescriptions. A total of 100 orders were placed with each of three online vendors that included a range of high- and low-powered single vision lenses and progressive addition lenses, and duplicate orders to assess repeatability. An independent certified testing laboratory was contracted to assess conformance with voluntary consensus standards (ANSI Z80.1-2015) and Food and Drug Administration drop-ball safety testing. Lenses not meeting these standards were counted as failures.
RESULTS
The overall failure rates for the three vendors were 11.2 ± 3.2% (vendor A), 8.0 ± 2.7% (vendor B), and 8.2 ± 2.8% (vendor C). The repeatability for 20 prescriptions ordered five times from each vendor was high, with correlation coefficients greater than 90%. There were no observed lens impact failures.
CONCLUSIONS
Almost 1 in 10 spectacle lens prescriptions ordered from three online vendors failed to meet national standards for optical quality. Additional studies are needed to assess eyewear ordered online for other important patient-specific variables that can influence visual performance and ultimate acceptability of prescription eyewear, such as lens placement relative to the visual axis, frame fit, and cosmetic acceptability.Permalink : https://bibliotheque.helb-prigogine.be/opac_css/index.php?lvl=notice_display&id= [article]Exemplaires

Miller Fisher syndrome, a variant of Guillain-Barré syndrome, is a condition characterized by ophthalmoplegia, ataxia, and areflexia. Diplopia, particularly secondary to a bilateral abduction deficit, is the most common presenting symptom. The telltale neurologic symptoms associated with this condition can easily be overlooked by eye care providers, delaying timely diagnosis and treatment.
PURPOSE
This study aimed to report a case of diplopia secondary to an uncommon condition (Miller Fisher syndrome) and to highlight the eye care provider's role in helping with diagnosis and management of this condition.
CASE REPORT
A 31-year-old woman presented to the emergency eye care service because of a 2-day history of sudden-onset diplopia, for which no cause was found 1 day prior at a local hospital emergency department. She also reported weakness in her legs, difficulty walking, balance problems, and reduced sensation of her left hand for the past 2 days. Clinical testing revealed bilateral abduction deficits, ataxia, and areflexia, the combination of which suggested Miller Fisher syndrome. Because of the acute onset and progressive severity of her neurologic symptoms, she was referred to a different hospital emergency department for confirmatory diagnosis and treatment of Miller Fisher syndrome.
CONCLUSIONS
Diplopia is a symptom commonly encountered by eye care providers, regardless of their mode of practice. Although there are many potential etiologies of diplopia, performing a comprehensive eye examination combined with a neurologic evaluation can potentially pinpoint the specific cause. Miller Fisher syndrome is one such condition in which the diagnostic triad can be uncovered with in-office ocular motility testing and neurologic examination. Although the incidence of glioblastoma is relatively low, patients presenting with this malignant central nervous system tumor will typically experience visual symptoms. Eye care providers are uniquely positioned to assist with the diagnosis of glioblastoma and to manage its associated visual symptoms.
PURPOSE
This case series will review the pathophysiology, diagnosis, treatment, and management of glioblastoma while highlighting the eye care provider's role over the course of the disease process.
CASE REPORTS
Three patients with glioblastoma are presented: a 63-year-old White man with new onset of headache and blurred vision upon awakening, a 65-year-old White man with a weeklong history of peripheral vision deficit and neurological symptoms, and a 65-year-old White woman with visual field deficit and visual perception difficulties related to a known history of glioblastoma.
CONCLUSIONS
It is incumbent upon eye care providers to recognize examination findings such as new neurological symptoms and/or visual field defects that may warrant additional workup.
