[Thésaurus Mesh]ThérapeutiqueMots-clés : angiogenèse Résumé : Background: Adhesions after tendinopathy in individuals who perform physical work and those physically active in middle age are a challenging problem for orthopedic surgeons. We evaluated the effects of human-derivated amniotic membrane on tendon healing, adhesions, angiogenesis, and the inflammatory process.
Methods: Thirty-five rats were divided evenly into five groups, and the left lower extremity was used in this study. No interventions were applied to the control group (group 5). In the other groups, Achilles tendons were partially cut to the midline. Then, primary repair (group 1), amniotic membrane treatment with no repair (group 2), primary repair and amniotic membrane treatment (group 3), or secondary healing with no repair (group 4) was performed.
Results: Use of amniotic membrane in tendon healing resulted in decreased adhesion formation and positive effects on collagen sequencing and anti-inflammatory effects. In addition, for the vascular endothelial growth factor evaluation there was no difference among the amniotic membrane repair groups, but there was an increase in vascular endothelial growth factor positivity compared with the control group.
Conclusions: These data show that amniotic membrane treatment can alter biological behavior and induce surface-dependent angiogenesis and can have angiogenetic effects on ischemia and inflammation.Permalink : https://bibliotheque.helb-prigogine.be/opac_css/index.php?lvl=notice_display&id= [article]Exemplaires

When conservative treatments are unsuccessful, the traditionalopen resection, debridement, and reattachment of the Achilles tendon is a variablyreliable procedure with significant risk of morbidity. Fasciotomy and surgical tenotomyusing ultrasound-guided percutaneous microresection is used on various tendons in thebody, but the efficacy has not been examined specifically for the Achilles tendon.Methods:A retrospective review evaluated 26 procedures in 25 patients who underwentAchilles fasciotomy and surgical tenotomy. The Foot Function Index was used toquantify pain, disability, activity limitation, and overall scores.Results:Mean Foot Function Index scores were as follows: pain, 8.53%; disability,7.91%; activity limitation, 2.50%; and overall, 6.97%. Twenty index procedures weresuccessful, and two patients repeated the procedure successfully for an overall 84.6%success rate in patients with chronic insertional pathology with mean surveillance of 16months. There were no infections or systemic complications.Conclusions:Ultrasound-guided percutaneous microresection is a safe and minimallyinvasive percutaneous alternative that can be used before proceeding to a more invasiveopen procedure. [Thésaurus Mesh]FémurRésumé : L’agence nationale d’appui à la performance des établissements de santé et médico-sociaux annonçait en 2013 une augmentation de la chirurgie ambulatoire de 42 % en orthopédie [1]. La chirurgie ambulatoire dans les arthroplasties de hanche, notamment par voie antérieure, n’échappe pas aux progrès technologiques et contribue à l’essor de la chirurgie rapide et du retour à l’autonomie précoce notamment grâce à la mise en place de programmes éducatifs pré et postopératoires.Le syndrome douloureux du grand trochanter (SDGT) confère une qualité de vie similaire à celle des patients souffrant de coxarthrose à un stade avancé. La majorité des patients sont observants de leur programme éducatif ; néanmoins, il arrive parfois que certains ne recouvrent pas la totalité de leur autonomie antérieure, se plaignant de douleurs localisées dans la région du grand trochanter.Souvent étiqueté comme une tendinopathie du moyen fessier, les programmes rééducatifs se montrent parfois insuffisants malgré des stratégies bien menées au regard d’une tendinopathie. Face à ce constat, l’analyse de la littérature et notre expérience nous ont permis de mettre en évidence un diagnostic différentiel de la tendinopathie du moyen fessier dont souffrent les patients après arthroplastie de hanche par voie antérieure, ce que les anglo-saxons appellent le syndrome douloureux du grand trochanter.
