The Valsalva maneuver is known to cause pre-retinal hemorrhages, which have a predilection to affect the macula. Certain conditions may increase the risk of developing Valsalva retinopathy including anemia. Knowing these associations can aid in uncovering underlying systemic conditions for which a patient may not otherwise be aware.
PURPOSE
This case report reviews the presentation of Valsalva retinopathy and the impacts of acute anemia on retinal findings. Recommendations for management are also reviewed and reported.
CASE REPORT
A 30-year-old Hispanic man presented to the emergency department for progressing weakness over the previous month. He had a history of polysubstance abuse and had not been eating for the previous month except for water intake. He was diagnosed with unconjugated hyperbilirubinemia, anemia, thrombocytopenia, hypercoagulable state, hypokalemia, and hypomagnesia and admitted to the intensive care unit for treatment and observation. He was referred to the eye clinic to be examined for Kayser-Fleischer rings because of concern for Wilson disease. On ophthalmological examination, the patient had reduced vision in both eyes attributed to bilateral subinternal limiting membrane macular hemorrhages likely due to Valsalva, with acute anemia and thrombocytopenia as contributing factors. The patient was referred to a retinal specialist, and observation was recommended with follow-up in 3 months. The patient, unfortunately, never followed up despite multiple attempts to reschedule.
CONCLUSIONS
Patient history and the common location of retinal hemorrhages aid in diagnosing Valsalva retinopathy. In this case, the patient had a known history of acute anemia and thrombocytopenia with a recent history of straining. Anemia, among other systemic conditions, can increase the risk of developing retinopathy after a Valsalva maneuver. Although Valsalva retinopathy can occur in healthy patients without underlying conditions, it is important to rule out other conditions that might present with retinal hemorrhage or that may increase the risk of Valsalva retinopathy.Permalink : https://bibliotheque.helb-prigogine.be/opac_css/index.php?lvl=notice_display&id= [article]Exemplaires

This case highlights various clinical features and resolution of Purtscher-like retinopathy after childbirth.
PURPOSE
This study aimed to present a case report of postpartum Purtscher-like retinopathy and subsequent resolution after oral and intravitreal treatment.
CASE REPORT
A 30-year-old woman presented with subjective visual disturbance and transient scotoma in her right eye 7 days after giving birth. Vision measured 20/20 in her right eye and 20/20 in her left eye. Clinical examination revealed optic disc edema, retinal ischemia, and retinal hemorrhage in the right eye with a grossly normal appearance of the left eye. A diagnosis of Purtscher-like retinopathy was made. Neuroimaging and bloodwork were noncontributory to the diagnosis. Approximately 1 month after treatment with rapidly tapered oral methylprednisolone and a single intravitreal bevacizumab (Avastin; Genentech, San Francisco, CA) injection, the retinopathy had resolved with subjective visual improvement.
CONCLUSIONS
Purtscher-like retinopathy can occur after childbirth, usually within 1 to 2 days postpartum and occasionally associated with preeclampsia. Clinical course and visual improvement vary, but novel treatments may aid in visual recovery.Note de contenu :

A travers de ce travail de fin d’études, je voulais comprendre, analyser les différentes répercussions au niveau visuel, engendré par la prématurité voire la grande prématurité.
La partie théorique aura pour intention de définir ce qu’est la prématurité, ses différents stades ainsi que les causes de celle-ci. Un chapitre relatera l’évolution du globe oculaire et du système visuel. Une partie traitera des réflexes primitifs et de l’incidence de la prématurité sur ces derniers.
La partie aura quant à elle pour finalité d’illustrer, à travers la littérature, des études concernant les conséquences visuelles d’une naissance dite trop tôt ; elle se clôturera par un questionnaire, point de comparaison entre la théorie et la réalité.
Enfin à travers ce travail, j’avais également un objectif plus personnel : mes connaissances et avoir une vision plus large et plus objective de ce qu’est « ma » prématurité (photo ci-dessous). Non-immunocompromised people living with HIV (NIPLHIV) share a similar immunocompetence to non-HIV (HIV) people with an elevated cluster of differentiation 4 (CD4) count in the era of antiretroviral therapy (ART). This has reduced the incidence of HIV retinopathy, thus raising the question of the integrity of the retinal morphology in NIPLHIV with longer life expectancy.
PURPOSE
The study assessed the retinal thickness and volume of NIPLHIV without retinitis on ART and attempted to find associations with linear parameters of cerebral atrophy.
METHODS
The study was conducted at a public hospital in South Africa. All people living with HIV were on ART with CD4 counts above 350 cells/mm3 and viral loads less than 10,000 copies/mL. The Heidelberg Spectralis ocular coherence tomographer assessed the retinal thickness and volume for comparison between 30 NIPLHIV and 30 HIV-negative participants. A subset of the HIV group obtained a computed tomography scan to determine the bicaudate ratio and the sylvian fissure ratio to assess cerebral atrophy. Independent t tests were performed to identify differences in retinal thickness and volume. Multivariate linear regressions measured associations between retinal thickness and volume with cerebral atrophy.
RESULTS
The NIPLHIV group had a thicker mean global temporal subfields at Early Treatment Diabetic Retinopathy Study (ETDRS) 3 mm (P = .047) and ETDRS 6 mm (P = .03). The mean global temporal subfield volume at ETDRS 3 mm was also increased in the NIPLHIV group (P = .02). Nasal macula retinal nerve fiber layer thickness and the inferior inner nuclear layer macula volume were directly related to the bicaudate ratio, whereas the volumes at the outer retinal layer subfields of the macula were inversely related to sylvian fissure ratio in NIPLHIV.
CONCLUSIONS
Macula thickness and volumetric differences do exist in NIPLHIV. Practitioners should keep NIPLHIV under retinal morphometric surveillance because they live longer.
