Elle peut entrainer des conséquences graves comme l’énophtalmie et la diplopie. Une reconstruction orbitaire anatomique est primordiale pour restaurer la fonction oculaire et l’esthétique faciale. Le titane est un matériau de choix dans cette indication. L’adaptation d’une plaque d’ostéosynthèse à l’anatomie complexe du plancher en peropératoire est la principale di fficulté chirurgicale. Pour contourner cel le-ci, il est intéressant de préformer la plaque en préopératoire sur un modèle orbitaire 3D. Cette étude réalise une revue systématique sur le sujet et propose un protocole innovant, simple et accessible. Matériel et méthodes : Une revue systématique examinant 5 bases de données a identifié un total de 288 résultats. Parmi ceux-ci, 24 articles ont été sélectionnés pour une analyse approfondie et 11 publications ont finalement été retenues. Un nouveau protocole utilisant cette technique est décrit. Résultats : Cette revue systématique met en évidence les avantages qu’offre cette technologie pour le traitement chirurgical des fractures de plancher d’orbite, notamment une amélioration de la précision des reconstructions chirurgicales, une réduction de la durée opératoire et moins de morbidité postopératoire. Conclusions : La technologie d’impression 3D offre de nombreux avantages dans la prise en charge des fractures du plancher orbitaire. Pour développer son expansion, nous proposons un protocole sûr et accessible utilisant cette technologie exploitant uniquement des logiciels gratuits. Methods: Sixty-five patients who were followed for at least 12 months and were older than 18 years were included. The posterior malleolus fractures were classified according to the Haraguchi classification system with computed tomography (CT). The posterior malleolus fragments were fixed with a plate through a posterolateral approach. Intraarticular step-off, reduction of the posterior malleolar fragment, and fibular position in the incisura were evaluated by early postoperative CT. American Orthopaedic Foot and Ankle Society (AOFAS) score and visual analog scale pain score were used for clinical assessment. Results: The posterior malleolus fractures were classified as Haraguchi type 1 in 45 patients (69.2%), Haraguchi type 2 in 12 patients (18.5%), and Haraguchi type 3 in eight patients (12.3%). No patients showed signs of instability or loss of reduction on direct radiographs during follow-up. Postoperative CT showed no loss of reduction in the posterior malleolus and tibiofibular alignment. On evaluation, there was no intra-articular step-off (,1) in any of the patients. The mean AOFAS score was calculated to be 91.6. The mean visual analog scale score was 1.2. Conclusions: We conclude that direct posterior fixation with the posterolateral approach can be a good option for ankle fractures involving posterior malleolar fragments.

The Evans osteotomy is a widely used procedure for the correction of adult and pediatric flexible flatfoot deformity. Locking plates are commonly used to stabilize the osteotomy and the allograft. However, there have been incidences of soft-tissue irritation caused by the hardware, requiring subsequent hardware removal. Therefore, we sought to review whether age, sex, or laterality of the procedure had any correlation with the rate of hardware removal.
Methods
A retrospective review was performed of 47 consecutive patients who underwent an Evans calcaneal osteotomy between October 1, 2013, and October 1, 2016. Data were collected and analyzed based on age, sex, laterality, and the need for hardware removal.
Results
All of the 47 patients met the inclusion criteria. Seventy procedures were performed, and hardware removal was required in 16 patients and 21 feet (30%). The only statistically significant finding was that 11 females and only five males required either unilateral or bilateral hardware removal (P = .039). All 16 patients reported complete pain relief after hardware removal.
Conclusions
Females are twice as likely as males to develop symptoms after locking plate application over an Evans osteotomy and may require hardware removal. Despite the low-profile nature of the locking plate to fixate the Evans osteotomy, the hardware can be a source of significant pain.
