[Thésaurus Mesh]Paralysie cérébraleRésumé : La paralysie cérébrale est la principale cause du handicap moteur de l’enfant. Les lésions cérébrales surviennent sur un cerveau en développement, elles ne sont pas évolutives mais définitives. Les déficiences liées à la posture et au mouvement étant au premier plan, les enfants atteints de paralysie cérébrale ont alors, le plus souvent, besoin de kinésithérapie tout au long de leur vie. Or, au cours de leur parcours de soin jalonné par de nombreuses interventions multidisciplinaires, ces jeunes perdent parfois leur motivation et le sens de leurs séances de kinésithérapie. Proposer une rééducation autrement, sous forme d’activités physiques attractives, ludiques et valorisantes nous permettrait d’être plus pertinents dans nos pratiques et de nous inscrire dans les recommandations de promotion de l’activité physique chez ces patients. To investigate the influence of time taken to begin musculoskeletal rehabilitation on injury recurrence and ankle-related medical care use at 1 year after ankle sprain.
Design
Retrospective cohort study of all beneficiaries of the US Military Health System seeking care for an ankle sprain over a 4-year period.
Methods
Individuals were classified according to whether they did or did not receive physical rehabilitation. For those who received rehabilitation (n = 6150), linear relationships (with appropriate covariate controls) were analyzed with generalized linear models and generalized additive models to measure the effects of rehabilitation timing on injury recurrence and injury-related medical care use (costs and visits) at 1 year after injury. The nonlinear effect of rehabilitation timing on the probability of recurrence was assessed.
Results
Approximately 1 in 4 people received rehabilitation. The probability of ankle sprain recurrence increased for each day that rehabilitation was not provided during the first week. The probability of ankle sprain recurrence plateaued until about 2 months after initial injury, then increased again, with 2 times greater odds of recurrence compared to those who received physical rehabilitation within the first month. When rehabilitation care was delayed, recurrence (odds ratio [OR] = 1.28), number of foot/ankle-related visits (OR = 1.22), and foot/ankle-related costs increased (OR = 1.13; up to $1400 per episode).
Conclusion

OBJECTIVES: To compare pain and disability in individuals with persistent nonspecific low back pain who were treated with Pilates exercises compared to minimal or other interventions. METHODS: Searches of Medline, CINAHL, Embase, Cochrane library, PEDro, and ProQuest Dissertations and Thesis databases were conducted. Randomized controlled trials (RCTs) were selected and reviewed if they compared pain and disability in individuals with persistent nonspecific low back pain who were treated with Pilates exercises compared to other treatment approaches. Quality of the trials was evaluated. Data for pain and disability scores were extracted. Narrative synthesis plus meta-analyses were performed, with either a fixed-effects or random-effects model, standardized mean differences (SMDs), and tests for heterogeneity. RESULTS: Seven RCTs were identified and included in the meta-analyses. Data pooling was performed using RevMan 5. When compared to minimal intervention, Pilates-based exercise provided superior pain relief (pooled SMD, –2.72; 95% CI: –5.33, –0.11; P = .04) but the pooled disability scores were not significantly different (pooled SMD, –0.74; 95% CI: –1.81, 0.33;P = .17). No significant differences were found when comparing Pilates-based exercise to other forms of exercise for pain (pooled SMD, 0.03; 95% CI: –0.52, 0.58; P = .92) or disability scores (pooled SMD, –0.41; 95% CI: –0.96, 0.14; P = .14). CONCLUSION: Pilates-based exercises are superior to minimal intervention for pain relief. Existing evidence does not establish superiority of Pilates-based exercise to other forms of exercise to reduce pain and disability for patients with persistent nonspecific low back pain. However, the relatively low quality of existing studies and the heterogeneity of pooled studies in this systematic review combine to suggest that these results should be interpreted with caution.
