Atteintes du nerf optique
Conditions which produce injury or dysfunction of the second cranial or optic nerve, which is generally considered a component of the central nervous system. Damage to optic nerve fibers may occur at or near their origin in the retina, at the optic disk, or in the nerve, optic chiasm, optic tract, or lateral geniculate nuclei. Clinical manifestations may include decreased visual acuity and contrast sensitivity, impaired color vision, and an afferent pupillary defect.

[Thésaurus Mesh]Syndrome de nécrose rétinienne aigüeMots-clés : ranibizumab traitement antiviral Résumé : SIGNIFICANCE: Acute retinal necrosis (ARN) may occur after intravitreal ranibizumab (IVR) treatment for patients
with exudative age-related macular degeneration (AMD). Awareness of this unusual but devastating complication
after IVR is needed. Early identification may help provide timely antiviral treatment and prevent irreversible visual loss.
PURPOSE: This study aimed to report a case of ARN after IVR in a patient with exudative AMD.
CASE REPORT: A 67-year-old male patient complained of blurred vision in his left eye for 1 month. The patient was
diagnosed with exudative AMD after detailed ophthalmic clinical evaluations. He received IVR once in his left eye.
Three days after IVR, he developed varicella-zoster virus–associated ARN, which was treated with systemic and intravitreal antiviral therapy. Because of progressive inflammation, the patient underwent 25G pars plana vitrectomy
with silicone oil tamponade. Seven months later, the patient was administered intravitreal aflibercept once in his
left eye. Three months after intravitreal aflibercept, he underwent removal of silicone oil, and retinal detachment
occurred 2 weeks after the surgery because of low IOP, and the patient eventually discontinued treatment.
CONCLUSIONS: This study reports the first case of varicella-zoster virus–associated ARN after IVR. Early ARN may
be very difficult to distinguish from intraocular inflammation after IVR. Therefore, early detection of viral DNA in
the intraocular fluid using polymerase chain reaction is recommended. Immediate antiviral treatment may be beneficial to prevent severe visual loss.Permalink : https://bibliotheque.helb-prigogine.be/opac_css/index.php?lvl=notice_display&id= [article]Exemplaires

[Thésaurus Mesh]Syndrome de Gougerot-SjögrenMots-clés : Atrophie Résumé : SIGNIFICANCE: Acute syphilitic posterior placoid chorioretinitis (ASPPC) is a rare clinical presentation of ocular syphilis. The worldwide incidence of syphilis has recently increased. Clinicians should be familiar with the distinct appearance and imaging findings of ASPPC and promptly initiate antibiotic treatment to limit functional vision loss and systemic complications of neurosyphilis. PURPOSE: This case demonstrates the utility of optical coherence tomography angiography (OCT-A) imaging to further understand the pathophysiology and visual prognosis of ASPPC. CASE REPORT: A 75-year-old man presented with 7 days of decreased vision in his left eye. His visual acuity was 20/20 in the right eye and 20/200 in the left eye. Fundus examination revealed placoid lesions in the superior and inferotemporal aspects of the posterior pole in the right eye and a large placoid macular lesion in the left eye. Optical coherence tomography imaging revealed disruption and loss of the ellipsoid zone and external limiting membrane, nodular elevations on the retinal pigment epithelium, and choroidal hyperreflective punctate lesions in the left eye. Optical coherence tomography angiography displayed choriocapillaris perfusion flow voids greater in the left eye than in the right eye. Two months after penicillin treatment, the placoid lesions resolved in both eyes with an improvement in outer retinal structural abnormalities on optical coherence tomography imaging and visual acuity to 20/25 in the left eye. Eleven months after presentation, the OCT-A choriocapillaris flow voids had improved without complete restoration. [Thésaurus HELB]:Paramédical:dronédaroneRésumé : SIGNIFICANCE: Amiodarone and dronedarone have recognized ophthalmological adverse effects including optic
neuropathy. The recognition of optic neuropathy as a complication of amiodarone and dronedarone treatment
may enable withdrawing the drug and accordingly preventing permanent vision loss.
PURPOSE: This study aimed to describe a case of optic neuropathy after substitution of amiodarone with
dronedarone for treatment of atrial fibrillation.
CASE REPORT: An 81-year-old man treated with dronedarone for atrial fibrillation after amiodarone had caused
tremor developed sequential permanent vision loss in both eyes.
CONCLUSIONS: The importance of timely recognition of optic neuropathy as a complication of amiodarone and
dronedarone treatment may enable discontinuing the drug, thus preventing permanent vision loss.Note de contenu : Idit Maharshak, MD, PhD,1,2 Michael Paul, MD,1,2 and Hillel Steiner, MD2,3*
